Before Your Visit

Bring the following documents to your appointment:

  • Identification: 

A valid ID like a New York State driver's license, or an American passport.

  • Proof of income: 

Pay stubs, an employer letter, or your recent tax return.

You may also need to provide proof of immigration status for non-citizens,

Do I need an appointment?

You do not need an appointment to receive help enrolling in a health plan. All walk-ins are welcome.

When can I come in to enroll?

Our health insurance case managers are here Monday - Thursday, 10:30 AM - 4 PM to help you. The office is closed for Jewish and federal holidays.

Which insurance plans do you help me enroll in?

We can help you enroll in all plans offered on the Marketplace. This includes:

Medicaid 

Child Health Plus 

Essential Plan 

Quality Health Plans 

What if I want a different plan?

Nachas Health has an on-site insurance broker to help you enroll in any plan not offered on the Marketplace.
